Appointments are typically made on a referral
basis only. Please provide the name of your referring dentist or
physician when calling to schedule an appointment so that
Associated Oral & Maxillofacial Surgeons of Peoria may contact
your doctor's office and obtain any essential documentation.

schedulers can be reached during working hours, 8:30 a.m. - 4:30
p.m. Monday through Thursday and 7:30 a.m. to 3:30 p.m. on Friday

Patient Registration
You may preregister with our office by filling out our secure
online Patient Registration Form. After you have completed the
form, please make sure to press the Submit button at the bottom to
automatically send us your information. On your first visit to our
office, we will have your completed form available for your
signature. The security and privacy of your personal data is one of
our primary concerns and we have taken every precaution to protect
it.
